G4TV has a new 12 minute video up on their site that outlines some of the things that Realism mode adds to the game while playing the campaign Hard Rain. Examples include being able to ditch a health pack for a defibrillator, getting up close to weapons to get them to highlight, increasing the number of witches and requiring zombie headshots to put them down. It is also mentioned that this mode isn't necessarily harder than other modes, it is just for people who play together often.

So far we've only found the boomer bile once and when I went to go throw it at a witch a common infected got in the way.
It was still pretty cool watching them all start fighting.
Umm, the defibrillators are in normal mode as well. I'm quite certain melee weapons can damage teammates.
So... all that "realism mode" offers is more witches and an emphasis on headshots? Really? That's not a "mode" that is a toggle. This marketing bullshit trying to cram another bullet point on the box is getting irritating.
What's next Smoker, Hunter, and Spitter modes? You're overwhelming me with your lack of inspiration and creative vision, Turtle Rock.
Anyway, despite the Doug Lombardi marketing bullshit, L4D2 has been pretty fun so far. It is irritating that the demo lacks Versus, but at least the weapon and engine upgrades are nice. Definitely not a sequel, but I preordered a 4-pack, so I think $30 is a fair price the the L4D expansion.
Yutt, in terms of the defibrillators, when you go down in realism mode, you stay down... there's no three strikes and your out policy. You'll need a defibrillator to revive someone who goes down so you can carry either a med kit or defib, not both.
